Medical Access

Our Medical Access Program includes a network of over 400 private health professionals that donate their time and services in order to provide care to uninsured and underinsured families living in the Seacoast of New Hampshire. Through our volunteer network, our participants have access to primary care, specialty care, dental care, chiropractic, mental health and holistic health services. Participants enrolled in SeaCare pay $10 - $20 for each office visit.
  • Physicians and Nurses: Participants receive care from volunteer physicians and their staff. Providers see participants in their offices and accept a small fee, usually $10, as the only payment.
  • Therapists: Mental health providers also donate time to treat specific needs of our participants for $10 per session.
  • Chiropractors:Participants seeking chiropractic services can receive care through the network for a small fee.
  • Holistic Providers: Reiki, Accupuncture, Naturopathic Medicine and other modalities are availible.

Care Coordination

Care Coordination is a service provided through a personal relationship between a participant and a knowledgeable staff member. Our Care Coordinators play an important role by supporting our participants in a variety of ways as they acquire the necessary skills to become managers of their own health.

Our Care Coordinators:
  • View each participant as a whole person, not simply someone with a medical situation.
  • Educate participants about available sources of care and how best to choose among them.
  • Teach participants how to be effective advocates for their own health care.
  • Address a participant’s emotional barriers to care.
  • Work with participants to identify resources in the community.
  • Provide a link to mental health services.
  • Provide a link to emergency dental services.
  • Assist participants with obtaining prescription assistance.
  • Work with participants to develop an action plan to achieve their personal goals.
  • Provide ongoing support by maintaining regular contact with their participants.
  • Are there when participants need a personal contact to help with problems.
  • Help participants to become knowledgeable consumers of health care once they leave SHS.
  • Provide an essential link between the community and the uninsured.



SeaCare Health Services ® Children's Connection is a home visiting program that assists families in accessing health care and promotes growth and development in children. The Children's Connection helps to educate and motivate families to create better health habits and to create meaningful, lasting relationships with their medical providers.

Resource Referrals
Care Coordination

Children’s Connection works closely with area physicians to ensure that your family receives complete and optimal care. We are able to assist parents understand how to make informed decisions regarding their children’s health.

Dealing with the Stress of Parenting



Medication Bridge is a program to help eligible uninsured and underinsured patients of all ages receive needed prescription medications.

Every major pharmaceutical company has a patient assistance program that provides prescription medications to eligible patients at no cost or a small co-payment.

SeaCare Health Services ® will work with your provider to apply for these medications.

The Medication Bridge Program at SeaCare is fully supported by Exeter Health Resources, who provides funding each year to cover the operating costs for this service. Since May 2001, the Medication Bridge Program has successfully dispensed more than 30,000 prescriptions totaling $6.5 million dollars to over 2,800 individuals.
