Children's Connection

About the Program

SeaCare Children's Connection is a home visiting program that assists families in accessing health care and promotes growth and development in children. The Children's Connection helps to educate and motivate you to create better health habits and to establish meaningful, lasting relationships with your medical providers.

Children’s Connection works closely with area physicians to ensure that your family receives complete and optimal care. We assist parents in understanding how to make informed decisions regarding their children’s health.

Services provided by the Children’s Connection begin with a family assessment. Once the needs are determined, we will then work with your family to identify goals and create a care plan. Services include:

Home-based Care Coordination Services

  • Linkage & referrals to agencies to assist families in building community resources and relationships
  • SeaCare Children’s Connection is a NH Healthy Kids Application Assistance Site. We will assist eligible families with NH Healthy Kids applications & documentation gathering
  • Assign a Primary Care Physician for children if necessary

Parenting Support

  • Child Safety
  • Child Development
  • Stress Management
  • ADHD Behavior Modification
  • Positive Discipline
  • Teaching Ethics and Values

Health Education

  • Asthma Management
  • Health Management and Prevention

Eligibility

Any family, regardless of income, is eligible to receive services from the Children’s Connection. Please contact the Children's Connection staff at 603-772-8119 if you are outside our service area. The staff will travel anywhere within a reasonable distance from our Exeter office.
