SeaCare Care Coordination

SeaCare’s professional care coordination staff is ready to join your health care team.

Our care coordinators assist you in obtaining a wide array of medical services.  We will help you navigate “the system” and act as your advocate.  Together, we create an action plan based on your social, physical and mental health priorities and support you along the way as you accomplish your goals.  We will connect you to programs, services and resources.  Your care coordinator will work with you in four distinct ways…

Care: we will assist you in gaining access to medical care

You will be assigned to a professional care coordinator. Your care coordinator will provide access to Primary Care Physicians and hundreds of specialty care providers.

If you have a Primary Care Doctor who is a SeaCare Provider, you may be able to continue with that doctor. If your doctor is not part of the SeaCare network, or you do not have one, you will be assigned to a new Primary Care Physician (PCP).

After you become established with your doctor or practice, you can be seen for any medical concerns you have for a small co-pay (usually about $10-$15 per visit). The doctor may determine that you need to see a specialist. SeaCare has a large number of specialty care providers who work with us. You will be asked to contact your care coordinator to access these specialists.

Coordinate: the plan, the people and the path

The care coordinators view each participant as a whole person, not simply someone with a medical situation. The initial intake interview will review your personal needs including family, housing, financial, transportation, educational and employment situations.

You will work with your care coordinator to develop an action plan for how to achieve your personal goals. While some goals are simple to achieve, such as arranging a routine physical examination, others may be more complex. The action plan includes a list of what needs to happen, prioritizes your goals, creates a time line and makes sure that each goal is successfully accomplished.

If you are unfamiliar with your rights and responsibilities as a medical patient in a private office, health center or hospital; we will help you learn what your responsibilities are and what you can ask providers to do for you. Care coordinators maintain regular communication with you and are your contact if you encounter any problems.

Connect: finding the right resources, programs and services for you

As we understand your situation, we can tell you about resources, services and programs for which you may be interested. We will talk with you about which resources may best fit your needs and how to choose among them. The care coordinators can connect you with resources within your local community and beyond.

We may also connect you with two additional SeaCare programs to assist you:

SeaCare Medication Bridge provides assistance with applying for long-term medications that are free or low-cost to those who qualify.

SeaCare Children’s Connection helps families meet the needs of their children with in-home care coordination, help in applying for assistance and educational programs for parents.

Coach: partner with your own personal healthcare coach to realize your health goals.

Studies show that the more involved people are in their own health care the better their health outcomes are. We will work along with the rest of your health care team to help you achieve and maintain your highest possible level of health. Together we will find information and consider your options. Then you will set achievable goals, broken into reasonable action steps, for yourself. We understand that working towards health goals may not always be easy. We will stand with you, give you support along the way and celebrate your successes.
